When a patient and their family choose hospice care, they are cared for by a highly trained team of professionals. The team is composed of the patient's attending physician, as well as Hospice nurses, social workers, volunteers, a chaplain, bereavement counselors and the Hospice medical director. Along with the patient's primary care giver, this team of experienced professionals work to assure that the patient is comfortable and receives needed care while functioning at the highest level possible.
