Hospice Care Team remains true to its original mission of providing help, hope and comfort to our patients. Help comes from a well-trained, professional team of physicians, nurses, social workers, home health aides, chaplains and volunteers. We inspire hope by maintaining the highest possible level of independence within familiar surroundings, supported by family and friends. We provide comfort through pain relief and symptom control for our patients, and encouragement and support to the family through active bereavement counseling.
